6-Pay Executive Payment Agreement

This payment option enables you to purchase our full version Executive Mentorship program using our convenient 6-pay payment plan.  The price of the Executive program using the 6-pay plan is a non-refundable $3589.00.  With the 6-pay payment option, you are agreeing to make an initial payment in the amount of $1794.50 plus $28.50 S&H.  You are also agreeing to make an additional 5 monthly payments in the amount of $358.90.  Products are delivered via CD. 

Payment Schedule:

Upon purchasing the 6-pay payment plan for the Executive Mentorship program, you're entering into a 6 payment contract with the Disability Associates Corporation.  You're agreeing to purchase our Executive training and support program with a total price of $3589.00.  All payments must be received by the date indicated in your initial payment schedule.  If payments are not received by the dates indicated in your payment schedule, all access to the training and software materials will be suspended until payments are paid in full.  To order your Executive Mentorship program using the 6-pay payment plan, complete this form and submit.
