information and referral service for those thinking of applying or
who have applied and been denied Social Security disability
benefits. This page will explain our procedure
for submitting information about your case. Please read this
segment carefully before completing the case profile form. All
information provided will be held in strict confidence. We
do not share personal client information with any outside
source.

Application Assistance Network
What's a Case
Profile?
A case profile is nothing more than
information supplied by you about your medical condition. All case profile
information is kept absolutely confidential and is not shared with anyone other than
the representative who will assess your case. Once your case has
been reviewed by a representative, your data will be automatically removed from our system.
When to use case profile
Please make sure that you meet
the following criteria before completing our case profile form:
1) You are between the ages
of 1 month to 64 years of age.
2) You currently suffer from a severe medical or mental disorder.
3) You are currently receiving medical treatment for your
impairment.
4) Your impairment limits your ability to perform work.
5) You are thinking of applying or have applied and been denied
SSA disability benefits.
6) Your impairment is expected to last at least one year.
When
not
to submit profile
1) You're about to move or
change your phone number.
2) You're currently on
Social Security disability benefits.
3) You're currently
being represented by an attorney or disability advocate.
4) You're currently working full-time despite your impairment with no
plans to quit.
5) Your impairment does not significantly reduce your ability
to work.
6) You have submitted a previous profile less than six months ago.
7) Your impairment is a result of a crime or illegal action.
8) You have no permanent home address.
Any profile
submitted with the above
characteristics will be rejected from our database.
What to Expect
After completing our case profile
form, your case information will be placed into our database which is
accessible only to our practicing disability advocates. If your case
information appears valid, you will be contacted by a representative who
will interview you and perform a free case assessment. At
this point, you and the representative will have the opportunity to
determine if representation would help you acquire benefits. There is absolutely no obligation!
Case Profile Form Completion
We ask that those
using our service provide complete and truthful information. All profiles are reviewed prior to being placed into our database.
Your case will not be added to the database if required information
is missing.
Improve your chances:
Please be aware
that your case may or may not be reviewed or accepted by a representative.
Professional disability advocates are in short supply and there may be none
practicing in your area. If there are no advocates in your area, you
may be contacted by a representative from another city or state. You
can increase the chances of having your case reviewed by providing as much
information as possible. Be sure to
provide current contact information (phone, e-mail address) that can be verified. Generally,
impairments that cause serious physical or mental
restrictions are more likely to be reviewed and accepted by a disability representative.
Submit Only Once:
Submitting your
profile more than once in a six month period will decrease the
chance of having your case reviewed. Our database automatically
removes duplicate profiles, so adding your information twice will only serve
to delete your information. If you need help immediately, indicate
this in your profile. Your profile will remain in
our database for a period of at least six months or until it is
chosen by an advocate for review. Please be patient!
